Melasma & Pigmentation Treatment in Kota Kinabalu
Honest treatment with realistic expectations - because melasma took years to form and won't disappear in a fortnight.
A Message from Dr. Tan
Here's something most doctors won't say out loud: sometimes we can't tell you with certainty what your pigmentation is.
Because what's actually on your face is rarely one clean condition. It's usually a mixture -melasma with post-inflammatory marks on top, or melasma alongside sun damage that's been accumulating for twenty years. Real faces are messy. Textbook photos are not.
So we start with an honest assessment. Where we're confident, we'll name it. Where we're not, we'll say so - and explain why. Then we work together on a plan based on what's realistic, not what sounds impressive.
What Melasma actually is?
Melasma is a pigmentation condition where your pigment-producing cells become overactive. It usually appears symmetrically across the cheeks, forehead, upper lip or jawline.
Three things drive it: hormones (pregnancy, hormonal medication), UV light, and visible light — ordinary daylight that comes through windows and reflects off walls. This last one is why melasma in Malaysia is so stubborn.
Melasma takes years to form. It builds slowly through thousands of hours of accumulated light exposure. Any product or person promising results in two weeks is either misinformed or selling you something.
The goal: reduce and then control — not erase and be done. Realistic improvement begins at around three months. Then we shift into long-term maintenance.

Sunscreen is the core treatment
If you take one thing from this page: sunscreen is not the supporting act — it's the main treatment.
For melasma, we need protection against UV and visible light. That means SPF 50+, PA++++, and iron oxide (which is why these products are tinted). We use Derm Eden DD Cream in the clinic.
The rule: At least twice a day, every day — even if you're indoors all day. Every 2-3 hours if you're in direct sunlight. Simple, no negotiation.
Cutting back on sunscreen once your skin improves is how you lose the ground you gained. The improvement is the sunscreen working.
Treatment options
We offer two routes and you choose with full information:
With hydroquinone — Faster results. Prescription cream applied at night for about two months, then off it and into maintenance. Requires proper on-off structure to be safe.
Without hydroquinone — Slower, gentler. Uses azelaic acid, tretinoin, and tranexamic acid (topical and, where appropriate, oral). Completely legitimate option.

Why we don't offer laser or peels for melasma
Melasma isn't a pigment deposit waiting to be removed — it's an active, ongoing process. Remove the pigment and the cells simply make more. With melasma, there's a real risk of the treatment provoking things and leaving you worse. We'd rather be honest about what holds up over years.
